<br />Individuals with Special Needs Emergency Planning, Registration <br />and Map Locate Project Meeting <br /> <br /> <br />Wednesday, October 10, 2012 <br />1:30 pm <br />Room “E” <br />Essex County Civic Centre <br /> <br />1.MEMBERS PRESENT: <br /> <br /> <br />Phil Berthiaume, Emergency Management Coordinator, County of <br />Essex <br />Mike Sherwood, GIS, County of Essex <br />Sue White, Recording Secretary, County of Essex <br />Dan Metcalfe, Essex-Windsor EMS <br />Donna Girard, Canadian Red Cross <br />Jennifer Tanner, 211 <br />2.REVIEW REGISTRATION FORMS, DETAILS REQUIRED, CONSENT <br /> <br />FORMS AND PRIVACY ISSUES <br /> <br />Committee members reviewed the proposed Consent Form and <br />revamped single page form. Mr. Berthiaume also noted that the draft <br />two page form would be kept as a back-up in case it was found that <br />the one page form required more information. He stated that the <br />Essex County Accessibility Advisory Committee (ECAAC) had <br />suggested making the form one page with less information on it so <br />that it was easier for people to fill out. These forms would be brought <br />to the next meeting of the Essex County Accessibility Advisory <br />Committee for their approval. <br />3.PROPOSED CHANGE OF PROJECT NAME <br /> <br /> <br />Mr. Berthiaume advised that input received from the ECAAC suggested <br />that the term “Special Needs Disability Registry” was objectionable to <br />them. He noted that in the United States, the registry was commonly <br />referred to as Accessibility and Functional Needs Project Group <br />Registry. <br />4.NEXT STEPS AND MILESTONES <br /> <br /> <br />Mr. Berthiaume advised that the next step would be to obtain the <br />support of the ECAAC in adopting the proposed two new forms.
