- 2 - <br /> <br /> 3. Every application for a license to operate a <br />lodging house shall be submitted to the Clerk on Form 1 <br />attached hereto, together with the appropriate fee as <br />hereinafter set out, and such application shall include the <br />following documents and information. <br /> <br />(a) <br /> <br />(b) <br /> <br />(c) <br /> <br />(d) <br /> <br />(e) <br /> <br />(f) <br /> <br />(g) <br /> <br />name and residence address of applicant, <br /> <br />address of lodging house for which license is <br />sought, <br /> <br />in the event that the applicant does not intend <br />to reside in the said lodging house, the name <br />and signed consent on Form 2 attached hereto of <br />a person over eighteen years of age who will <br />reside in the said lodging house as agent of <br />the applicant for the purposes of this by-law, <br /> <br />maximum number of occupants and number of rooms <br />available for such purpose, <br /> <br />number of separate showers, bathtubs (with or <br />without showers), wash basins, water closets, <br />urinals and sinks in the premises, <br /> <br />particulars of meals to be served and location <br />and capacity of rooms to be used for dining <br />purposes, and <br /> <br />a floor plan in duplicate of such premises <br />showing measurements and designation of each <br />room, provided that it shall not be necessary <br />to submit such floor plan if the application is <br />for renewal of a license held in the previous <br />year for the same premises, and if such <br />application states that there has been no <br />change in the number, location, size and use of <br />the rooms in such premises since the time of <br />issue of the former license. <br /> <br /> 4. Upon receipt of an application for license, the <br />Clerk shall immediately advise the Medical Officer of Health <br />who shall then inspect the premises and arrange for <br />inspection also by the Building Inspector and the Fire Chief. <br />If the application complies with the provisions of this <br />by-law and if the Building Inspector, the Fire Chief and the <br />Medical Officer of Health approve the operation of a lodging <br />house on the said premises, the Medical Officer of Health <br />shall approve the application and shall so advise the Clerk <br />who shall issue a license forthwith. <br /> <br /> <br />
