THE CORPORATION OF THE COUNTY OF ESSEX BY -LAW NUMBER 46-2006 A BY -LAW AUTHORIZING THE EXECUTION OF A SERVICE AGREEMENT BETWEEN COLUMBIA LIFE REHABILITATION INC. AND THE CORPORATION OF THE COUNTY OF ESSEX FOR THE PROVISION OF PHYSIOTHERAPY SERVICES WHEREAS The Corporation of the County of Essex wishes to retain a Physiotherapy Service Provider to provide required physiotherapy services to the residents of the Sun Parlor Home; AND WHEREAS Columbia Life Rehabilitation Inc. agrees to provide the services of Physiotherapists who are licensed or accredited under a1] applicab]e laws and regulations and by all applicab]e authorities to provide physiotherapy services; NOW THEREFORE the Council of the Corporation of the County of Essex enacts as follows: ] . THAT the Warden and Clerk be and they are hereby authorized to execute a Physiotherapy Service Agreement between The Corporation of the County of Essex and Columbia Life Rehabilitation ]nc., and all other documents required to obtain designated OHIP physiotherapy funding, and that the acts of said officers of this Corporation who may have already executed and delivered the said Agreement are hereby ratified and confirmed as the acts of this Corporation. 2.
