STUDY SUBSITE AGREEMENT This.agreement ("Agreement") is made as of the last date of signature below ("Effective Date") by and between: ottawa Hospital Research lnstitute, research arm of rhe ottawa Hospital 725 Parkdale Avenue, Ottawa, Ontario K1y 4Eg (Hereinafter referred to as "INSTITUTION,) and Dr. Christian Vaillancourt The Ottawa Hospital (Civic Campus) Department of Emergency Medicine 1053 Carling Avenue, Ottawa, ON K1y 4Eg (Hereínafter referred to as' PRt NC I PAL I NVESTIGATOR) and The Corporation of the County of Essex Essex-Windsor EMS 360 Fairview Avenue West Essex, Ontario N8M 1Y6 (Hereinafter collectively referred to as "EMS") Hereinafter PRINCIPAL INVESTIGATOR, tNSTtruÏoN, and EMS may be referred to individually as a "Party" and collectively as the "parties". and Justin Lammers Essex-Windsor EMS 360 Fairview Avenue West Essex, Ontario N8M 1Y6 (Hereinafter referred to as "EMS INVEST|GATOR") EMS INVESTIGATOR, while not a Party or Partíes as defined above, is a signatory to the Agreement and has rights, responsibilities and obligations as specified in thè Agreêment. REGITALS: The PRINCIPAL INVESTIGATOR has designed a study ("Study") as set out in the protocot ("Protocol") titled 'A Pragmatic Strategy Empowering Paramedics to Assess Low-Rìsk Trauma Patients with the Canadian C-Spine rule and Selectively Transport them Without lmmobilization" incorporated herein by reference and listed in Exhibit "A'. The Study will be sponsored by the INSTITUTION and performed as part of a mutti-centre initiative and the Study Protocol is funded by an IMPACT award granted to the pRtNClpAL INVESTIGATOR, via the INSTITUTION, through the Ontario SPOR SUPPORT unit ("OSSU") which receives funding from the Canadian lnstitutes of Health Research ('CtHR). piincipal lnvestigator will be responsible for the overall conduct of the Study.
