STUDY SUBSITE AGREEMENT <br />This.agreement ("Agreement") is made as of the last date of signature below ("Effective Date") by <br />and between: <br />ottawa Hospital Research lnstitute, research arm of rhe ottawa Hospital <br />725 Parkdale Avenue, Ottawa, Ontario K1y 4Eg <br />(Hereinafter referred to as "INSTITUTION,) <br />and <br />Dr. Christian Vaillancourt <br />The Ottawa Hospital (Civic Campus) <br />Department of Emergency Medicine <br />1053 Carling Avenue, Ottawa, ON K1y 4Eg <br />(Hereínafter referred to as' PRt NC I PAL I NVESTIGATOR) <br />and <br />The Corporation of the County of Essex <br />Essex-Windsor EMS <br />360 Fairview Avenue West <br />Essex, Ontario <br />N8M 1Y6 <br />(Hereinafter collectively referred to as "EMS") <br />Hereinafter PRINCIPAL INVESTIGATOR, tNSTtruÏoN, and EMS may be referred to <br />individually as a "Party" and collectively as the "parties". <br />and <br />Justin Lammers <br />Essex-Windsor EMS <br />360 Fairview Avenue West <br />Essex, Ontario <br />N8M 1Y6 <br />(Hereinafter referred to as "EMS INVEST|GATOR") <br />EMS INVESTIGATOR, while not a Party or Partíes as defined above, is a signatory to the <br />Agreement and has rights, responsibilities and obligations as specified in thè Agreêment. <br />REGITALS: <br />The PRINCIPAL INVESTIGATOR has designed a study ("Study") as set out in the protocot <br />("Protocol") titled 'A Pragmatic Strategy Empowering Paramedics to Assess Low-Rìsk Trauma <br />Patients with the Canadian C-Spine rule and Selectively Transport them Without lmmobilization" <br />incorporated herein by reference and listed in Exhibit "A'. <br />The Study will be sponsored by the INSTITUTION and performed as part of a mutti-centre <br />initiative and the Study Protocol is funded by an IMPACT award granted to the pRtNClpAL <br />INVESTIGATOR, via the INSTITUTION, through the Ontario SPOR SUPPORT unit ("OSSU") <br />which receives funding from the Canadian lnstitutes of Health Research ('CtHR). piincipal <br />lnvestigator will be responsible for the overall conduct of the Study. <br />OHRI- Aug 2016 C-Spine Study 1of13
