CORPORATION OF THE COUNTY OF ESSEX BY-LAW NUMBER 22-2005 A BY-LAW TO AUTHORIZE THE EXECUTION OF AN AGREEMENT BETWEEN THE CHANGE FOUNDATION AND THE CORPORATION OF THE COUNTY OF ESSEX. WHEREAS the Sun Parlor Home, a senior residence providing comprehensive supportive-living care, has expertise in and infrastructure to support the development, implementation and evaluation of the nursing initiative identified as Innovation in Nursing Practice Project No: 05127; AND WHEREAS The Change Foundation (TCF) is a registered charitable organization having a mandate to promote, support and improve health and the delivery of health care through four activity areas being: Applied Research and Analysis, Development Programs, Education Programs and Grants to Charities; AND WHEREAS TCF will receive certain funding from the Ministry of Health and Long Term Care that it will use to pay part of the costs of work to be carried out by Sun Parlor Home; AND WHEREAS TCF wishes to enter into an agreement with the Sun Parlor Home whereby the Sun Parlor Home will agree to carry out a certain project entitled Nursing Guidelines for End-of--Life Care in Long Term Care Settings with a view to developing a care mapping guide for End-of--Life Care, resulting in improved nursing practice and enhanced satisfaction level for residents and their families. The results of this project will be shared by TCF and Sun Parlor Home; NOW THEREFORE the Council of the Corporation of the County of Essex enacts as follows: THAT the Warden and Clerk be authorized to execute and deliver on behalf of the Corporation of the County of Essex the said Agreement, and that the acts of said officers of this Corporation who may have already executed and delivered said Agreement are hereby ratified and confirmed as the acts of this Corporation. 2.
