<br />FAMILY SERVICES CREDIT COUNSELLING EAP. <br />PROPOSAL TO PROVIDE <br /> <br />Corporation of the County of Essex <br /> <br />WITH A FULL SERVICE EMPLOYEE ASSISTANCE PROGRAM <br /> <br />Covering the period April I, 2005 to March 31, 2006 with reference to Employee Assistance Program at <br />Corporation of the County of Essex. <br /> <br />Part A: <br /> <br />Family Services Credit Counselling Employee Assistance Program. (FSCC EAP) proposes to provide <br />Corporation of the County of Essex with employee assistance counselling services, as per the EAP. <br />Proposal and in accordance with Family Service's National Quality Assurance standards. Specifically, <br />Family Services Credit Counselling EAP (FSCC EAP) proposes to: <br /> <br />1. Provide program management/liaison, professional counsellors, and the support services of Family <br />Services Credit Counselling (FSCC) to perform the following functions: <br /> <br />a) to provide confidential counselling service to staff and members of their immediate families at any <br />of the three services with an assured response within 24 hours of initial contact and the offer of an <br />appointment within 48 business hours; <br /> <br />b) to provide confidential counselling service to employees and their immediate families for up to six <br />months after layoff or job loss; <br /> <br />c) to provide a referral service to community resources as required with appropriate follow-up where <br />needed; <br /> <br />d) to provide up to two workshops each year as determined by the ongoing needs of Corporation of <br />the County of Essex. <br /> <br />e) to coordinate the collection of data that will facilitate the evaluation of the service without <br />compromising the confidentiality of the program, reporting statistics to persons identified; <br /> <br />f) to perform any other activity that might be reasonably requested by the parties involved to ensure <br />the ongoing success of the service; <br /> <br />g) to maintain liability coverage at a level of no less that $1,000,0000.00 inclusive of bodily injury <br />and/or property damage per occurrence. <br /> <br />2. Provide 24-hour emergency service 1-800 telephone number to handle social and emotional crisis <br />of Corporation of the County of Essex staff and their immediate families. <br /> <br />3. Maintain a clearly neutral role in all matters affecting the relationship between Corporation of the <br />County of Essex and its employees. <br /> <br />4. FSCC EAP may at its option assign this agreement to another qualified employee assistance <br />counselling service provider, providing it gives at least two weeks notice to the Corporation of the <br />County of Essex. In the event of any such assignment FSCC EAP shall be released from its <br />obligations under this agreement. <br /> <br />5. Inform appropriate Corporation of the County of Essex officials, no later than January 1,2006, of <br />revisions to this agreement, including proposed fee changes and other matters that will require <br />decisions before the new contract year, which commences April I, 2006. <br />
