BY-LAW 3159-95 <br /> <br />A BY-LAW TO AUTHORIZE THE EXECUTION OF <br />AN AGREEMENT BETWEEN HER MAJESTY THE <br />QUEEN, IN RIGHT OF ONTARIO, AS <br />REPRESENTED BY THE MINISTER OF HEALTH, <br />AND THE CORPORATION OF THE COUNTY OF <br />ESSEX. <br /> <br />WHEREAS The Corporation of the County of Essex has jointly entered into <br />an Agreement with Her Majesty The Queen, in right of Ontario, as <br />represented by the Minister of Health, to operate a Program entitled <br />Friendly Visiting Program for isolated elderly or disabled persons provided <br />in the client's home or program office for immediate assistance in response <br />to an identified need. The program assists clients in coping with every day <br />tasks of living in the community. Services include: information and <br />referral; assisting in filling out forms; making application for various <br />services and programs; advice on nutrition, shipping household <br />maintenance; and social resources available in the community, as well as, <br />residential placement options. <br /> <br />NOW THEREFORE the Council of the Corporation of the County of Essex <br />enacts as follows: <br /> <br />THAT the Warden and Clerk be and they are hereby authorized to <br />execute an Agreement between Her Majesty the Queen, in right of <br />Ontario, as represented by the Minister of Health, and The <br />Corporation of the County of Essex, a copy of which is attached <br />hereto, and affix the Corporate Seal thereto; <br /> <br />THIS By-law shall come into force and take effect on the 18th day <br />of lanuary, 1995. <br /> <br />1st reading - Jan. 18/95 <br />2nd reading - Jan. 18/95 <br />3rd reading - Jan. 18/95 <br /> <br />CLERK "'~ <br /> <br /> <br />
