PARLORI~tE FOR SENIOR CITIZENS - <br />SUN <br /> <br /> MEMORANDUM OF ABREEMENT <br /> <br />CAL DIRECTOR <br /> <br />BETWEEN: <br /> <br />THE CORPORATION OF THE COUNTY OF ESSEX <br />(hereinafter referred to as "the Corporation") <br /> <br />and <br /> <br />DR. ROBERT PAGE <br /> <br />(hereinafter referred to as "the Medical Director") <br /> <br /> WHEREAS the Corporation operates a home for the aged known <br />as the Sun Parlor Home for Senior Citizens at 175 Talbot Street <br />East, Leamington, Ontario (hereinafter referred to as "the home") <br />and the Medical Director is a legally qualified medical <br />practitioner ~ho has agreed to provide medical services to the <br />residents of the home in accordance ~ith the requirements of <br /> <br />applicable legislation; <br /> <br />Now therefore in consideration of the sum of $1.00, other good <br />and valuable consideration and in consideration of the covenance <br />and promises contained in this Agreement the parties agree as <br /> <br />follows: <br /> <br />1.0 The Medical Director will be responsible for the <br />provision of medical care to residents of the home, including <br />those in Community Support Home(s) operated by the Corporation. <br /> <br />2.1 The Medical Director will adhere to the requirements of <br />the Homes for the Aqed and Rest Homes Act and the regulations <br />made thereunder as exemplified in Appendix 'A" to this agreement <br />and the policies established from time to time by the Ministry of <br />Community and Social Services. <br /> <br />2.2 The Medical Director will at his own expense keep up to <br />date with current knowledge and practice in geriatrics and <br />gerontology. To this end, membership in the Ontario Association <br />of Medical Directors of Homes for Senior Citizens is required. <br />The Medical Director will maintain membership, for which the <br />Corporation will reimburse the Medical Director the annual <br />membership fee. <br /> <br />2.5 The Corporation will reimburse the Medical Director for <br /> <br />actual expenses for registration, transportation and <br /> <br />accommodation incurred for attendance at one related medical <br />meeting~ annually, held in Ontario and approved by the <br /> <br />Corporation. <br /> <br /> <br />
